Palliative and End of Life care
Caring for resident's as they approach the end of their life is one of the most important and rewarding areas of care. Our ethos is to enable residents to die as they wish their needs are assessed, wishes and preferences discussed and an agreed set of actions duly implemented. Each resident has an individualised care plan which reflects the choices made.
We recognise that family members, close friends of residents approaching the end of life, have a vital role in the provision of care. We closely involved them in decision making, with the recognition that they also have their own needs. We provide information about the likely progress of the resident's condition we are also able to provide practical and emotional support both during the resident's life and after bereavement.
End of life care is challenging and indeed emotionally demanding for all involved- our staff have the necessary skills, knowledge and attitude to undertake this in an empathetic manner. Our homes use the Liverpool Care Pathway, which was first developed for use with cancer patients, and which has now been successfully modified for use for people with other conditions.
A number of our homes have achieved or are working towards the nationally accredited Gold Standard framework in End of Life Care.
